TRANSITIONAL LIVING PROGRAM

The CC’s Transitional Living Program (TLP) is a 24-month program that offers supervised or unsupervised housing with structure and holistic services that empower youth to transition to independence.

WHY JOIN

By joining our program, youth gain the tools and guidance needed to develop crucial life skills, enhancing self-reliance and resilience for a successful transition to adulthood.

AGE OF PARTICIPANTS

TLP-G serves youth ages 16-19. TLP-S is suitable for youth ages 18-22.

The difference between TLP-G and TLP-S

The CC offers two tracks for the Transitional Living Program (TLP) that cater to the unique needs of youth.

The CC welcomes youth in each track to The CC’s campus for an enriching 24-month journey to independence. Services provided include safe and structured housing, case management, life skills coaching, art therapy, educational support, access to the on-site nurse and nurse practitioner, transportation support, employment support, therapy, and more. After completing the TLP, youth may receive up to 6 months of continued counseling and case management services to ensure they’re well prepared to live independently.

TLP-G

TLP-G is structured and supervised group housing. TLP-G is designed for youth ages 16-19.

TLP-S

TLP-S is unsupervised, shared apartments. TLP-S offers less structure and more independence. TLP-S is suitable for ages 18-22.

Check Out The Hub

The members of our programs have full access to The Hub.

The Hub is a safe, supervised center for recreation and community offering comprehensive services for unhoused youth and youth at-risk of facing housing instability.

It is a place where youth ages 10-19 can come together to unlock their potential, learn valuable life skills and have fun with other youth.

What services and support are provided in the Transitional Living Program?
